Consent to the Processing of Personal Data

1. I hereby, acting freely, of my own will and in my own interest, give my consent to Sole Proprietor Alexander Anatolyevich Makarikhin (INN 773121379142, OGRNIP 322774600000104) (hereinafter — the Operator) to process my personal data provided by me in the booking form on the website makarikhin.com.

2. Consent is given to the processing of the following personal data: name; contact details (email address and/or messenger username, phone number); information voluntarily provided by me in the “Your concern” field.

3. Purpose of processing: contacting me regarding my booking for a consultation and agreeing on the date, time, and format of the appointment.

4. Processing includes the following actions: collection, recording, systematization, storage, updating, use, deletion, and destruction of personal data. Processing is carried out without transfer to third parties, except in cases provided for by the legislation of the Russian Federation.

5. This consent is valid until the purpose of processing is achieved or until it is withdrawn. I have the right to withdraw my consent at any time by sending a letter to the email address specified in the “Contacts” section of the website. If consent is withdrawn, the Operator ceases processing and destroys the data within a period not exceeding 30 days.

6. Personal data is processed in accordance with Federal Law No. 152-FZ of July 27, 2006 “On Personal Data” and the Privacy Policy published on the website.
